I scrolled through the report at work and the thing that jumped out was the lack of a single 3-D cad model used by all the contractors. Especially one that incorporates structural, HVAC, piping, electrical, equipment etc etc.
This has been pretty much the standard for the past 20 years in my industry. There is no way for you not to have massive rework and cost over runs otherwise.
In fact while I was reading the report I had the PDM S model open for a 40,000 ton topsides we had designed.
