The big take away from this article:

MU-90 had a difficult entry to service. In 1997, the navy decided it needed a new torpedo to replace 1970s era Mark 46 torpedoes. In 1999, defence chose the MU-90 in the mistaken belief this was a low risk "off-the-shelf" acquisition.

Also interesting is the pattern of waste. Australia, which has a small Defence force, now has two kinds of new light-weight torpedoes.
